Register your interest to become a PPI Forum Member or in Our Health Network and/or keep you updated with the work of the Commission. Alternatively click here to download and print off an application form in pdf format and then send it to your region or the national centre. Download a Word format version of the Equal opportunities monitoring form (this form is optional), or print it off and mail it to us with the application form.
